JOB
Summary
Works under the supervision of the Branch Nursing Leader (Director of Nursing, Nurse Manager, Director of Patient Care Services). This position is responsible for coordinating nursing care for infusion patients and monitoring the ongoing nurse/patient relationship and patient progress. The position will include establishing and maintaining positive relationships with patients, home health agencies and referral sources. The Nursing Clinical Coordinator will work closely with the intake department, clinical pharmacists, and administrative department.

Responsibilities
- Coordinates with intake and pharmacy departments to obtain appropriate clinical informationand coordinate nursing care for infusion patients.
- Ensures appropriate staffing for all patients.
- Works with clinical pharmacy team to communicate nursing/pharmacy concerns.
- Contacts patients as appropriate to introduce company and services, collect data andcoordinate scheduling.
- Coordinates all agency activities related to nursing including scheduling, direct bill andsubcontracted nursing communication, visit billing and the processing of invoices.
- Initiates patient entry and maintains patient visit schedules
- Works closely with nursing, pharmacy and warehouse to coordinate visit schedule needsensuring appropriate patient care.
- Tracks POTs and Nursing Orders
- Participates in KabaFusion’s performance improvement program activities including chartaudits, data collection and quality improvement plans.
- Serves as primary contact/liaison between KabaFusion branch and various home healthnursing agencies.
- Develops cooperative relationships with home health agencies, referrals sources, and allKabaFusion departments.
- Ensures timeliness of recerts, lab draws and all time-sensitive visit activities
- Other related duties as assigned
Skills and abilities
- Ability to organize and prioritize work with attention to detail
- Demonstrated experience in delivering customer service to both internal and external clients, ensuring adherence to quality service standards, and assessing customer satisfaction.
- Proven track record of cultivating and sustaining relationships within a team.
- Proficient in time management, including skills in prioritization, organization, and meticulous tracking of project details to meet multiple deadlines with varying completion dates.
- Competent in basic email operations, internet explorer usage, encompassing sending, receiving, and organizing communications.
- Possesses fundamental proficiency in Microsoft Word and Excel
Education and/or
Experience
- Active Licensed Vocational or Licensed Practical Nurse in the State assigned.
- 2 years' experience in a home health care or home infusion setting or case management experience preferred.
- Knowledge of, home health services, knowledge of intravenous therapies, ability to communicate both verbally and in writing to clearly gather information.
- Knowledge of third-party reimbursement preferred
