Trinity Health IHA Medical Group is seeking a compassionate and patient-focusedRegistered Nurse (RN) to join our outpatient care team. In this role, you'll help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience. The RN III works in a capacity that requires advanced training, education, certification and/or leadership. Additionally, RN III’s may have program administration and staffing development responsibilities.
What You'll Do
- Proficient to work within scope of nursing certification that includes, but is not limited to lactation consulting, advanced wound ostomy care, care/case management certification, diabetes educator or asthma educator certification.
- Serves as a nurse leader by assisting management team with hiring, motivating and engaging clinical staff.
- Identifies and targets at-risk patients who require extensive care to improve health outcomes.
- Assess and triage patient needs by phone and in-office visits.
- Provide clinical guidance, patient education, and care coordination.
- Support patients through transitions of care and post-discharge follow-up.
- Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
- Manage medication refills, test results, referrals, and prior authorizations.
- Identify preventive care needs and support chronic disease management.
- Help patients access community resources and navigate barriers to care.
- Serve as a clinical resource and leader within the care team.
What We're Looking For
Education
BSN strongly preferred or Graduate of an accredited Registered Nurse program. Advanced certification/training relevant to specialty (e.g., AAACN, IBCLC) or graduate degree preferred.
- CREDENTIALS/
Licensure
Valid, unrestricted RN license in the State of Michigan. Valid CPR certification.
- MINIMUM
Experience
3 years of clinical experience in a ambulatory care or specialty, and/or demonstrated competency in IHA’s RN II position.
