Responsibilities

• Provide direct patient care to residents in a long-term care setting, including routine assessments and assistance with activities of daily living
• Administer prescribed medications and treatments per physician orders; document administration accurately
• Monitor vital signs, assess changes in resident condition, and report findings promptly to the supervising nurse
• Maintain accurate charting and documentation in the electronic health record; ensure continuity of care
• Assist with wound care, basic IV care where applicable, and skin integrity; support mobility and fall-prevention efforts
• Collaborate with the interdisciplinary team to develop and implement care plans; communicate changes and concerns to the charge nurse or supervisor
• Uphold infection control policies, patient safety standards, and residents’ rights

Requirements

• Active LPN license (in good standing)
• Local candidates only for the Newport, Vermont area
• Ability to work night shifts (3x12 hours) and meet the stated schedule
• Comfortable working in a long-term care/skilled nursing environment with geriatric residents
• Ability to pass background check and credentialing requirements
