Position Summary
The Licensed Practical Nurse (LPN) serves as a key member of the clinical team by providing both front office and clinical support services. This position is responsible for patient check-in and intake, telephone triage and communication, chart preparation and review, and coordination of patient care. The LPN ensures efficient patient flow, outstanding customer service, and compliance with all clinical, regulatory, and organizational standards.
Essential Duties and Responsibilities
Patient Reception & Check-In
Greet patients and visitors in a professional, courteous, and compassionate manner.
Check patients in and verify demographic, insurance, and contact information.
Obtain and document required forms and consents.
Maintain patient confidentiality and HIPAA compliance at all times.
Telephone Management
Answer incoming phone calls promptly and professionally.
Triage patient calls according to established protocols and clinical judgment.
Document telephone encounters accurately in the electronic medical record (EMR).
Respond to patient inquiries and coordinate follow-up care.
Chart Review & Clinical Documentation
Review patient charts prior to appointments to ensure completeness and readiness.
Verify referrals, laboratory results, imaging reports, and outside records.
Identify care gaps and notify providers of outstanding clinical needs.
Maintain accurate and timely documentation in the EMR.
Clinical Support
Obtain and document vital signs, medical history, medications, allergies, and chief complaints.
Administrative & Operational Support
Maintain organized patient records and clinical documentation.
Assist with inventory management and ordering of supplies.
Participate in quality improvement initiatives and staff meetings.
Perform additional duties as assigned to support clinic operations and patient care.
Requirements:
Qualifications
Required
Current Licensed Practical Nurse (LPN) license in the State of Vermont.
Graduate of an accredited Practical Nursing program.
Current CPR/BLS certification.
Proficiency with electronic medical records (EMR) systems.
Strong organizational, communication, and customer service skills.
Ability to multitask in a fast-paced healthcare environment.
Preferred
Previous experience in an outpatient clinic, specialty practice, or physician office.
Experience with telephone triage and patient scheduling.
Knowledge, Skills, and Abilities
Excellent interpersonal and patient-service skills.
Strong attention to detail and accuracy in documentation.
Ability to prioritize tasks and manage competing demands.
Working knowledge of HIPAA, infection control, and patient safety standards.
Ability to work independently and collaboratively with providers and staff.
Proficiency in Microsoft Office and EMR software.
