We are seeking a dedicated and compassionate PRN (as-needed) Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to provide high-quality home health care visits. - Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation, to develop individualized care plans.
- Administer medications, including injections and IV infusions, following proper aseptic techniques and safety protocols.
- Monitor patients’ conditions closely, identifying changes or complications such as acute pain or infection, and escalate concerns appropriately.
- Provide education to patients and caregivers on disease management, medication administration, nutrition, and self-care techniques.
- Document all patient interactions accurately using Electronic Health Record (EHR) systems while maintaining strict compliance with HIPAA regulations.
- Coordinate with interdisciplinary teams to ensure continuity of care across various settings such as skilled nursing facilities, assisted living communities, or long-term care environments.
- Support clients with specialized needs including dementia care, Alzheimer's care, memory care, or individuals with developmental disabilities.
- Assist with complex procedures such as catheterization, tube feeding management, airway management, and specimen collection when necessary.
*Skills*
- Proven experience working in diverse clinical settings such as home health, hospital medicine, ICU, emergency medicine, or long-term care environments.
- Strong knowledge of medical terminology, ICD coding (ICD-9/10), and familiarity with EMR/EHR systems like Epic or Cerner.
- Expertise in critical care procedures including IV insertion, venipuncture, sterile processing, and infection control practices.
- Experience managing acute pain cases and providing urgent or emergency room-level interventions in a home environment.
- Ability to communicate effectively with patients of all ages—including pediatrics and geriatrics—and their families; demonstrated caregiver communication skills.
- Knowledge of long-term care regulations and best practices for skilled nursing facility operations.
- Proficiency in performing physical examinations, vital signs monitoring, medication administration, and health coaching.
- Familiarity with specialized areas such as hospice care, infectious disease management, trauma center protocols (Level I/II), neonatal ventilator use, or PICU experience is advantageous.
