*Job Summary*
We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) – Field Nurse to join our dedicated healthcare team. *Duties*
* Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation using electronic health record (EHR) systems such as Epic or eClinicalWorks.
* Administer medications, injections, IV infusions, and perform catheterizations following sterile techniques and aseptic procedures.
* Manage complex care plans for patients with chronic conditions like dementia, Alzheimer’s disease, or acute pain management needs.
* Provide wound care, tube feeding assistance, ventilator management, and airway support tailored to individual needs.
* Educate patients and caregivers on medication administration, disease management (including infectious diseases), nutrition, and safety protocols aligned with long-term care regulations.
* Document all patient interactions accurately in medical records while maintaining strict compliance with HIPAA and clinical confidentiality policies.
* Collaborate with interdisciplinary teams including physicians, case managers, and specialists in emergency medicine or ICU settings to coordinate seamless patient care.
* Respond promptly to triage calls or urgent situations by assessing symptoms and determining appropriate interventions or referrals.
* Support clients with developmental disabilities or those receiving hospice or palliative care through compassionate bedside manner and specialized interventions.
*Experience*
* Valid LVN license with current state licensure required; additional certifications such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) are preferred.
* Demonstrated experience working in diverse healthcare environments including home care, skilled nursing facilities, hospitals (ICU/trauma centers), or assisted living communities.
* Proven proficiency with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate documentation and patient data management.
* Critical care experience in settings like ICU or Level I Trauma Centers is highly desirable; familiarity with ICD-9/ICD-10 coding enhances clinical documentation skills.
* Experience providing dementia or Alzheimer’s care within memory or long-term care facilities is advantageous.
* Background in managing acute pain cases, infectious disease protocols, ventilator support, catheterization procedures, and tube feeding techniques is beneficial.
* Strong knowledge of physiology, anatomy, pharmacology related to medication administration and medical terminology is essential for delivering safe patient care.
