Responsibilities
Provide skilled nursing care and monitor patient status per LPN LTC standards.
Administer medications and document administration accurately in EMR.
Assess patient needs, update care plans, and communicate changes to the team.
Monitor and report vital signs, symptoms, and deviations promptly.
Document all care and interventions clearly to ensure quality and compliance.
Assist with daily living activities and maintain residents' comfort and safety.
Collaborate with physicians, RNs, and care staff to optimize outcomes.
Adhere to regulatory and facility policies; participate in quality improvement efforts.
Maintain high productivity while achieving targets and meeting performance metrics.
Identify opportunities for professional growth and participate in development programs.
Assist with admissions, discharges, and transition of residents between units.
Experience/Qualifications
Licensed Practical Nurse (LPN) in good standing with applicable LTC regulations.
Provide skilled nursing care and monitor resident status per LTC standards.
Administer medications and document accurately in the EMR system.
Assess needs, update care plans, and communicate changes to the care team.
Maintain thorough, compliant documentation of all care interventions.
Adhere to regulatory policies and participate in quality improvement initiatives.
Monitor vital signs and promptly report deviations.
Assist with daily living activities while ensuring residents’ safety and comfort.
Collaborate with physicians, RNs, and staff to optimize outcomes.
Demonstrate productivity and meet performance targets; manage own workload.
Identify growth opportunities and participate in professional development programs.
Assist with admissions, discharges, and inter-unit transitions smoothly.
On-site employment with contract status; flexible to meet shift needs.
Comfort with a growth-oriented, metrics-driven environment and high activity visibility.
