*Position Overview*

We are seeking a compassionate, motivated, and skilled *Physical Therapist (PT)* to join our expanding Home Health team. In this role, you will provide individualized rehabilitative care to patients in the comfort of their homes while helping them achieve functional goals, prevent decline, and improve overall quality of life.

*What You’ll Do*

  • Perform comprehensive patient evaluations and develop individualized Plans of Care
  • Deliver high-quality physical therapy interventions based on physician orders and patient goals
  • Help patients improve mobility, strength, balance, endurance, and safety
  • Educate patients and caregivers on exercises, fall prevention, and safe mobility techniques
  • Monitor patient progress and modify treatment plans as needed
  • Collaborate closely with physicians, nurses, case managers, and interdisciplinary team members
  • Maintain timely and accurate documentation in compliance with agency and state regulations
  • Participate in case conferences and contribute to quality patient outcomes
  • Supervise and mentor Physical Therapist Assistants (PTAs) when applicable
  • Advocate for patient independence, dignity, and overall well-being

*What We’re Looking For*

_Required Qualifications_

  • Current California Physical Therapist License
  • Valid Driver’s License
  • Reliable transportation with current auto insurance
  • Ability to travel to patient homes within assigned territories
  • Excellent communication and interpersonal skills
  • Ability to safely assist and transfer patients as needed

_Preferred Qualifications_

  • Home health experience strongly preferred
  • Experience in hospital or outpatient rehabilitation settings
  • Bilingual skills are a plus
  • Experience using EMR systems

  • California PT License
  • Driver’s License & Auto Insurance
  • TB Test or Chest X-Ray
  • Physical Examination
  • CPR Certification

Pay: $80.00 - $110.00 per hour
