Provides telephonic triage and document review services that require clinical judgement and interpretation and acts as a liaison between the patient, care team(s), and their primary care provider. Reviews and triages telephone encounters, calls, and messages from patients. Prioritizes urgent medical triage. Reviews lab results, imaging results, consultation notes, and clinical summaries. Coordinates team based care in the absence of the Program Manager. Requires RN licensure in Rhode Island and 3-5 years of nursing experience.
