Registered Nurse (Transfer Coordinator)

The Registered Nurse Transfer coordinator is directly responsible for the facilitation and monitoring of the community hospital referrals to the VA facility, VA to VA, and VA to community hospital. This individual is substantially responsible for collaborating with other staff for appropriate oversight and management of emergency care received through community hospitals. Duties include executing position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex client care. Works with providers to coordinate clinical and administrative information needed to make prudent decisions regarding services, providing accurate, timely and comprehensive analysis and reports to VISN 9 leadership and VA leadership. Has oversight of the emergency reporting system for emergent admissions to community hospitals. Follows Code of Federal Regulations (CFR) when reviewing emergency episodes of care for 1703, 1725, and 1728. Collaborates with Care in the Community, House Supervisor, Traveling Veteran Coordinator, and Bed Control.
