Home Health RN
Responsibilities
- Conduct initial and ongoing patient assessments in the home setting
- Develop and update individualized care plans in collaboration with the care team
- Administer medications, treatments, and injections as prescribed
- Perform wound care and dressing changes and other complex care
- Monitor vital signs and symptoms, reporting changes to the physician
- Educate patients and caregivers on disease processes, medications, and self-care
- Coordinate care with physicians, therapists, and other healthcare professionals
- Document all visits and care provided in the EMR
- Ensure compliance with agency policies, Medicare/Medicaid, and other regulations
- Participate in case conferences and interdisciplinary team meetings
- Provide emotional support and promote patient independence
Requirements
- Washington RN licensure
- Home Health/Hospice agency experience preferred, but not required
- Current BLS certification
- Valid driver’s license and insured vehicle
