*Job Overview* We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to join our healthcare team on a PRN (as-needed) basis. This position offers flexible scheduling options, allowing you to make a meaningful impact whenever your skills are needed most. *Duties* * Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems. * Administer medications and injections accurately following physician orders, including catheter and wound care. * Manage acute pain and provide specialized care for patients with dementia, Alzheimer's disease, or memory impairments in memory or long-term care sett * Document all patient interactions thoroughly in compliance with HIPAA regulations and medical documentation standards. * Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans, discharge planning, and case management strategies. *Skills* * Proven experience working with diverse populations including geriatric patients, or those requiring hospice or palliative care. * Strong knowledge of medical terminology, anatomy, physiology, and pharmacology related to acute and chronic conditions. * Proficiency in EMR/EHR systems such as Cerner, Epic, Athenahealth, or eClinicalWorks for accurate documentation and communication. * Skilled in sterile technique to ensure patient safety. * Excellent caregiver communication skills with the ability to educate patients and families effectively about treatment plans and health coaching. * Ability to perform physical examinations, vital signs monitoring, intake assessments, and triage efficiently. * Knowledge of long-term care regulations including skilled nursing facility standards and working with geriatric or pediatric populations.
