About this role

The Community Care Coordinator Registered Nurse is responsible for executing a streamlined approach to directing timely, coordinated care for Veterans. Serves as a liaison to internal and community providers, manages Veteran care throughout the consult process, and uses nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options including internal/direct VA care and community care. Possesses knowledge of internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, and VHA standards.

Requirements

  • U.S. Citizenship
  • Current, full, active, and unrestricted RN license in a State, Territory, or Commonwealth of the U.S. or District of Columbia
  • Graduate of accredited nursing school (ACEN or CCNE)
  • English language proficiency
  • Associate Degree (ADN) or Diploma in Nursing minimum; BSN, MSN, or Doctoral degree depending on grade level
  • Background/security investigation
  • Drug screening (urinalysis)
  • Pre-employment physical examination
