Registered Nurse (RN) / Licensed Practical Nurse (LPN) – Private Duty

We are recruiting *RNs and LPNs* who want flexible, well-paid, private-duty work with families who specifically want a licensed nurse—not only a CNA or home health aide.

Families choose to work with an LPN or RN when they want the added knowledge, clinical judgment, and experience that a licensed nurse brings to home care. Assignments may involve medication awareness, wound and skin care, post-surgical recovery, changes in condition, or more complex care needs. If you enjoy hands- care and want to continue using your nursing skills and license in a home setting, Cary’s Circle offers the flexibility to choose assignments that fit your experience and interests.

Responsibilities

*Two ways to work (you choose what you accept)*

• *Private-duty caregiving shifts. *Direct care in the client’s home (or recovery setting) for families who want licensed-nurse coverage. Shifts may be days, evenings, overnights, weekends, short-term recovery, or ongoing private duty. You accept only the assignments that fit your schedule.
• *Nursing assessments, caregiver training, and clinical consultation. *In-home visits to assess needs, advise on the right level of care, and train caregivers on client-specific skills (including wound/skin care and other home-care skills).

_Backup coverage when a caregiver calls out is available if you want it. It is never required._

*What you may do on a caregiving assignment*

• Provide hands-on personal care and licensed nursing support within your scope
• Monitor condition, skin integrity, mobility, transfers, fall risk, and cognition
• Support wound/skin care, drains, catheters, ostomies, oxygen, or similar home-care needs when they are part of the assignment and within your license
• Help families after hospital discharge or surgery, including post-surgical recovery when requested
• Give medications only as allowed by your license, the care arrangement, and applicable Maryland rules
• Document clearly and communicate professionally with the family and Cary’s Circle
• Recognize changes that need family notification or medical follow-up
• Work independently and keep professional boundaries

*What you may do on an assessment/training visit*

• Complete an in-home assessment before services start, after a change in condition, or when reassessment is needed
• Identify clinical issues that affect caregiver matching
• Train CNAs, HHAs, or family caregivers on specific skills
• Help a family understand what level of care is appropriate
• Provide observations and recommendations within your nursing scope

_Cary’s Circle does __*not*_ _employ you to supervise another caregiver’s ongoing care. When you assess or train, you are providing professional nursing assessment, education, consultation, and/or skills training. When you accept a caregiving shift, you are providing that care as an independent licensed nurse._

*Required*

• Current, unrestricted Maryland RN or LPN license (or compact privilege valid in Maryland)
• At least 3–5 years of nursing experience
• Experience in home care, private duty, geriatrics, senior living, skilled nursing, rehab, med-surg, or similar
• Experience with older adults and/or people with complex care needs
• Wound care training and/or significant wound care experience
• Strong assessment, communication, and clinical judgment
• Comfort working independently in private homes
• Reliable transportation
• Professional liability/malpractice insurance appropriate for independent practice
• Current BLS (ACLS a plus)

*Strongly preferred*

• Interest in regular or recurring private-duty shifts, not only one-time visits
• Experience training caregivers, CNAs, HHAs, or nursing staff
• Dementia and behavioral support experience
• Transfers, fall prevention, catheters, feeding tubes, oxygen, ostomies
• Post-surgical or hospital-to-home recovery experience
• Comfort working with families who expect a high-touch, discreet standard of care

This is not employment with Cary’s Circle. You maintain your license, insurance, transportation, schedule, and professional practice. You decide which assignments to accept. You are responsible for working within your Maryland scope of practice.
