Nurse

• Perform comprehensive initial and ongoing patient assessments in the home setting.
• Develop, implement, and update individualized care plans based on patient needs and physician orders.
• Provide skilled nursing services (e.g., wound care, medication management, disease management, injections, catheter care, etc.).
• Educate patients and caregivers on diagnoses, medications, treatments, and safety measures.
• Monitor patient progress and report significant changes to the physician and clinical supervisor.
• Coordinate care with interdisciplinary team members (case managers, therapists, caregivers, and other healthcare professionals).
• Complete accurate and timely documentation in accordance with agency policies and regulatory requirements.
• Ensure compliance with all applicable state regulations, Medicare/Medicaid guidelines (if applicable), and agency standards.
• Participate in care conferences and quality assurance activities as needed.
• Maintain patient confidentiality and adhere to HIPAA regulations.

• Active and unrestricted Registered Nurse (RN) license in [State].
• Minimum [1–2] years of clinical nursing experience; home care experience preferred.
• Strong assessment, critical thinking, and clinical judgment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and valid driver’s license.
• Basic computer skills and experience with electronic medical records (EMR) preferred
