Responsibilities
Provide compassionate in-home health support to clients with daily living tasks.
Conduct routine health assessments and document patient status and progress.
Administer prescribed medications and monitor for adverse reactions.
Ensure safety compliance and maintain professional standards in all visits.
Document all activities accurately in electronic health records.
Create and update care plans in collaboration with supervisors and family.
Track patient outcomes using measurable metrics and pipelines.
Coordinate with interdisciplinary team to ensure seamless care delivery.
Manage time and route planning to maximize visit efficiency and productivity.
Educate patients and families on health management and discharge planning.
Demonstrate initiative for continuous learning and skill development.
Experience/Qualifications
Licensed practical nurse (LPN) or registered nurse (RN) with active state license.
Ability to perform accurate health assessments and document progress in EHRs.
Medication administration and adverse reaction monitoring experience.
Current CPR/First Aid certification and infection control training.
Experience delivering in-home health care or community health services.
Competence in care planning, updating plans with family and supervisors.
Strong time management and route planning to maximize visit productivity.
Effective interdisciplinary collaboration and communication skills.
Keen familiarity with outcome tracking, metrics, and pipeline-based planning.
Customer/family education skills and discharge planning competence.
