About this role

Nature and Scope

The Minimum Data Set (MDS) Coordinator works in conjunction with physicians, other nursing staff, residents, families and other facility departments to assure that the resident assessment tool, resident care plan and interdisciplinary team meeting are completed according to all Federal/State regulations and according to facility policy/procedure in order to provide optimum individualized nursing care for the residents of SNF service.

Compensation

Compensation Philosophy

  • We have 13 pay increments on our clinical pay scales to compensate staff for 30 years of relevant experience.

Compensation range

  • RN $60.61 - $86.42
  • LVN: $36.74 - $52.38

Job Status / Shift Information

Per Diem, non-benefited - Remote or In-person

Qualifications

Education

  • Must possess, as a minimum, a Nursing Degree from an accredited college or university
  • Must possess a RN or LVN California License
  • Current CPR

  • Working the floor and performing daily RN duties.
  • Ideal candidate must be a skilled communicator, director, and motivator
  • Able to organize and prioritize many tasks effectively

Child Care

  • PDH is thrilled to provide our Child Care Center services to all families in our area. Our program is operational from 7:45am to 5:15pm, Monday through Friday, and is approved for children aged 0 to 5 years. After submitting your application, our Child Care Director will get in touch with you about your acceptance into the program. Once approved, we will start the enrollment process. PDH employees take priority on our waiting list. For any additional questions please contact our Child Care Center Director at 530-283-7165 or childcare@pdh.org
