Responsibilities

  • Provide compassionate and quality care to hospice patients and their families
  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Administer medications and treatments as prescribed by the physician
  • Monitor and evaluate patient's response to treatment and adjust care plan as needed
  • Educate patients and families on disease process, symptom management, and end-of-life care options
  • Provide emotional support and counseling to patients and families
  • Coordinate with other healthcare professionals to ensure continuity of care
  • Document patient care activities accurately and timely

Qualifications

  • Valid RN license in the state of Illinois
  • Minimum of 1 year of nursing experience, preferably in hospice or palliative care
  • Knowledge of infection control principles and practices
  • Familiarity with sterile processing techniques
  • Experience in family planning and counseling
  • Ability to lift heavy objects and assist with ambulatory patients
  • Familiarity with dementia care techniques
  • Knowledge of medical imaging procedures and interpretation
  • Experience with esthetic laser treatment is a plus
  • Familiarity with durable medical equipment (DME) usage and maintenance
  • Proficiency in sonography is a plus
