Anchor Health is seeking an After-Hours Hospice LVN/ RN to provide skilled, on-call nursing care outside of standard business hours.

Position Overview
The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM.
The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.
Schedule
- 5pm-8am
- Weekends and holidays as assigned
- On-call/after-hours coverage
Key Responsibilities
- Respond to after-hours patient calls and urgent care needs
- Perform skilled nursing assessments and symptom management
- Provide emotional support and education to patients and families
- Implement and update patient plans of care as needed
- Communicate patient condition changes to the interdisciplinary team
- Collaborate with physicians and caregivers regarding patient needs
- Complete accurate and timely clinical documentation
- Uphold hospice philosophy, policies, and regulatory standards
Qualifications & Requirements
Current RN or LVN license in good standing Hospice or home health experience preferred Strong clinical judgment and ability to work independently Excellent communication and problem-solving skills Compassionate presence in emotionally sensitive situations Valid driver's license Reliable, insured vehicle in good working condition (per state and organizational requirements)
