CNA / RN / LVN

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
• Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations.
• Assist with activities of daily living such as bathing, dressing, feeding, mobility support, and hygiene for diverse patient populations.
• Monitor patients’ conditions closely, recognize signs of acute pain or deterioration, and escalate concerns promptly using triage protocols.
• Maintain detailed medical documentation within Electronic Health Records (EHR) systems like Epic or Cerner while ensuring compliance with HIPAA and confidentiality policies.
• Support specialized care needs including dementia care, Alzheimer's care, memory care, hospice support, and infectious disease management.
• Collaborate with multidisciplinary teams in acute care settings such as Level I Trauma Centers or Level II Trauma Centers, providing critical nursing interventions in fast-paced environments like ICUs or emergency rooms.

*Skills*

• Proven experience working with geriatric patients in skilled nursing facilities or long-term care settings; familiarity with ICD-9/ICD-10 coding is a plus.
• Strong knowledge of medical terminology, anatomy, physiology, and clinical procedures such as venipuncture, sterile processing, airway management, and tube feeding.
• Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or similar platforms for accurate medical records management.
• Skilled in patient assessment techniques including vital signs measurement and diagnostic evaluation to inform treatment plans.
• Ability to perform specialized tasks such as IV insertion/infusion therapy, catheter care, phlebotomy, and first aid with aseptic technique.
• Experience working with individuals with disabilities or developmental delays; familiarity with dementia and memory care strategies enhances patient support.
• Excellent caregiver communication skills to effectively coordinate with families and healthcare teams while maintaining clinical confidentiality.
