Title: APP Telehealth Clinical Supervisor
Department: Personalized Health Partners (PHP)
Reports to: Director of Clinical Operations
Position Summary
The NP Clinical Supervisor plays a critical leadership role in supporting the delivery of high-quality patient care, optimizing provider performance, and ensuring operational excellence across multiple healthcare settings, including but not limited to skilled nursing facilities, assisted living communities, independent living communities, and telehealth services. This role will be 40% Administrative & 60% Providing Care.
Location: Fully Remote. Must reside in United States. Company schedule & operations on Eastern Time Zone. See below for required state licensure.
Schedule:
Nights and (Alternating) Weekends; 40 hours per week

Primary Responsibilities and Essential Functions
Supervise and provide real-time leadership to clinical staff, including Nurse Practitioners, Registered Nurses, and Physicians.
Manage providers, system workflows, and clinical operations to support efficient provision of telehealth services including timely completion of visits, documentation, orders, and follow-up activities.
Monitor real-time provider productivity, and quality of care to ensure support achieving organizational and team goals.
Provide ongoing coaching, mentoring, and training to clinical team members. The NP Clinical Supervisor serves as a clinical resource and mentor while supporting the organization's strategic initiatives and value-based care goals.
Assist with onboarding, orientation, and training of newly hired providers and clinical staff.
Conduct regular chart audits and clinical documentation reviews to ensure compliance, accuracy, quality, and integrity of care.
Follow up on clinical reports, quality measures, audit findings, and performance improvement initiatives.
Ensure the delivery of high-quality patient care and compliance with organizational policies, professional standards, and regulatory requirements.
Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR) system.
Support business strategies and operational initiatives that align with PHP clinical and financial goals.
Oversee provider scheduling to ensure appropriate clinical coverage and staffing needs are met.
Review and approve timekeeping records for assigned staff in accordance with organizational policies.
Coordinate provider access requests, credentialing support, and EMR access requirements upon hire and as providers obtain additional state licensure.
Support strategic initiatives related to value-based care, quality improvement, patient outcomes, and population health management.
Collaborate with clinical leadership to identify operational challenges and implement solutions that improve efficiency and patient care.
Practice ethically and in accordance with the Scope and Standards of Practice for the profession and applicable board certification standards.
Adhere to all federal, state, and local regulations, laws, policies, and guidelines.
Serve as a resource for clinical and administrative issues, addressing concerns and escalating matters as appropriate.

Support provider recruitment efforts, onboarding processes, and retention initiatives.
Assist leadership with special projects and operational initiatives as assigned.
Education and Qualifications
Required Qualifications
Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduate of an accredited Nurse Practitioner Certificate Program.
National certification through AANP or ANCC.
Active and unrestricted state license(s).
Ability to obtain additional state licenses within the service area as required (telehealth providers may be required to maintain licensure in OH, IN, WV, MD, VA).
Active DEA license.
Authorization to work in the United States.
Minimum of two (2) years of clinical experience as a Nurse Practitioner in a geriatric, adult, or family practice setting.
Ability to work in a flexible and evolving healthcare environment.
Demonstrated passion for delivering healthcare services to senior and underserved populations.
Preferred Qualifications
One (1) or more years of leadership, supervisory, or clinical management experience.
Training or clinical experience in geriatrics.
Three (3) or more years of Nurse Practitioner experience.
Experience with coding, ICD-10, and Hierarchical Condition Categories (HCC).
Experience conducting chart audits and quality reviews.
Experience working within a metrics-driven, results-oriented healthcare model.
Willingness to pursue ongoing professional development, certifications, and continuing education.
Experience working in value-based care models.
Experience in skilled nursing, assisted living, long-term care, or post-acute care settings.
Demonstrated success working in collaborative healthcare environments to improve outcomes and achieve organizational goals.
Experience in outpatient primary care settings.
Experience utilizing EMR systems, provider scheduling platforms, and payroll systems.
Knowledge, Skills, and Abilities
Strong verbal and written communication skills.
Exceptional leadership, mentoring, coaching, and team-development capabilities.
Strong knowledge of EHR systems, clinical documentation standards, billing and coding practices, and quality review processes.
Thorough understanding of provider workflows, clinical operations, and regulatory compliance requirements.
Detail-oriented with the ability to work independently and collaboratively.
Demonstrates a customer-service-focused approach with patients, facility partners, and team members.
Strong organizational and time-management skills with the ability to prioritize competing demands and meet deadlines.
Ability to analyze performance metrics and implement improvement strategies.
Ability to effectively manage staff schedules, provider access requests, and operational priorities.
Ability to lead change, manage multiple priorities, and drive accountability within clinical teams.
