Patriot Health and Rehabilitation Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our growing team in Paris, Tenessee. In this role, you will provide direct resident care, administer medications, and support our clinical team in delivering high-quality care in a skilled nursing and rehabilitation facility.
Hiring for full time night shifts (7p-7a)!
New grads welcome! Are you looking to gain skills with Peg tubes,Trach care, IV med administration, lab draws or Rapid Response Teams? Do you want to learn more about IV antibiotic therapy and Infectious Disease Management? Our talented nurse mentors will guide you to help improve your assessment and critical thinking skills.

LPN Requirements
- Active Licensed Practical Nurse (LPN) license in Tennessee
- Experience in long-term care, skilled nursing, or rehab preferred
- Strong clinical and assessment skills
- Excellent communication and teamwork abilities
- Ability to prioritize and work in a fast-paced environment
- Willingness to work weekends and holidays as needed
LPN Responsibilities
- Provide direct nursing care according to individualized care plans
- Administer medications and treatments as ordered
- Monitor resident condition including vital signs and changes in status
- Complete accurate and timely documentation
- Assist with admissions, transfers, and discharges
- Work closely with RNs, CNAs, physicians, and therapy staff
- Support residents with daily care needs when needed
- Participate in care planning and family meetings
- Follow infection control and safety procedures
- Help train and support new nursing staff
