About this role
Legacy Living Care LLC is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to an elderly client in her home.
The client has dementia and is considered a fall risk, with a need for medication management and ongoing safety monitoring.
Key Responsibilities
- Provide skilled nursing care according to the client's plan of care and provider orders.
- Medication management, including medication administration/oversight and monitoring for effectiveness, side effects, or concerns.
- Fall-risk and safety monitoring, including assessment of mobility, balance, and changes that may increase fall risk.
- Monitor the client's cognitive and overall condition and report significant changes to the appropriate healthcare provider.
- Provide basic caregiver/family education regarding medications, safety, and changes in condition.
- Complete accurate nursing documentation and communicate concerns or changes in condition to Legacy Living Care and the treating provider.
Qualifications
- Current, unrestricted Georgia LPN license.
- Current CPR/BLS certification.
- Experience with dementia/Alzheimer's care preferred.
- Experience with medication management and fall-risk patients preferred.
- Dependable, compassionate, and comfortable working independently in a home-care setting.
