Department Description: Do you desire to work independently and autonomously, and have the full support of an amazingly talented interdisciplinary team at your fingertips? Are you passionate about hospice, educating clients and families and enjoy variety?
We are looking for a community hospice admission nurse who will be the first point of contact for our clients and families as they enter onto hospice and will ensure that the experience is personal and individualized while honoring specific client goals.
The admission RN will provide comprehensive coordination of admissions beginning with collaboration with our intake department, reviewing referrals for eligibility and then will schedule and complete in person admissions. You will collaborate with the client and caregivers around hospice plan, while organizing and directing the initiation of hospice care along with the associated charting and care coordination before handing off to the RN case manager and assigned interdisciplinary team members.
Minimum
Requirements:
• Minimum of four years of experience as Community Hospice Nurse with a strong job performance record, or minimum of five years of experience in home health nursing, or a combination of other education and experience from which comparable knowledge and skills are acquired.
• Certification in Hospice and Palliative Care preferred.
On-call: Not typically, variable dependent upon program needs.
Incentives: $10,000 Sign On Bonus for full time external applicants, prorated for part time roles! Additional terms and conditions apply. Mileage reimbursement at Federal rates.
Requirements:
Current RN licensure recognized by the State of Vermont required.
Appropriate experience in specific clinical area. Varies by unit.
