Qualifications

  • Graduate of accredited school of nursing, currently registered in New York State as a Registered Professional Nurse. Life support and infection control certificate required. • At least 1 year experience as a staff nurse preferably in an ambulatory care setting preferred. New grads with 3 months med/surg experience required. • Recent experience performing phlebotomy. • Ability to work independently and part of a team; prioritize tasks. Must have the ability to interface with participants/patients as well as all levels of staff. • Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial and ethnic backgrounds. • Must possess excellent organizational and communication skills. • Bi-lingual skills in English and Spanish are preferred. • Computer literacy with proficiency in Microsoft Word. Experience documenting in an EMR • Excellent verbal and written communication skills. Excellent and interpersonal skills. Bi-lingual in English and Spanish helpful. Position

Summary

  • Assumes responsibility and accountability for the provision of nursing care in accordance with relevant federal requirements, state and local laws and nurse practice acts, regulatory standards of professional ambulatory care nursing practice, other relevant professional standards, and organizational policies. • Coordinates, implements, evaluates and directs patient care activities, appropriate to the age of patients served and areas of clinical practice, in collaboration with members of the health care team. • Supports the mission and philosophy of Damian Family Care Centers, Inc.

Essential Functions

Demonstrates the ability to perform assessments and data collection in a systematic manner, focusing on physiological, psychological, cognitive, age specific and cultural needs of the patients. Consistently utilizes current evidence based standards of professional practice and standards of care in the assessment and planning process. Demonstrates initiative, competency and enthusiasm for carrying out daily assignments; assisting and taking direction from other health care providers in the delivery of patient care and maintaining patient flow.

Collaborates effectively with medical providers and other clinical disciplines in planning interventions that will best meet the patient and family needs in a timely manner.
