About this role
Overview
Coordinates patient care across the acute care continuum by partnering with physicians, nursing, social services, and ancillary teams to develop and implement an interdisciplinary plan of care. Conducts admission and ongoing assessments, monitors clinical progress and resource utilization, and leads safe transitions of care, including discharge planning and referrals to community services. Educates patients and families, addresses psychosocial and social determinants of health barriers, and collaborates with Utilization Management regarding level of care considerations.
Serves as the central communicator to promote collaboration, continuity, and achievement of defined outcomes such as reduced readmissions and optimized length of stay.

Responsibilities
- Monitors admission and continued stay of all patients regardless of level of care.
- Monitors avoidable days which are delays in treatment or timely patient discharge, patient care inefficiencies, and/or barriers to treatment and/or discharge.
- Updates patient file (PCP, insurance, etc.) to ensure timely discharge
- Collaborates with patient’s physician regarding discharge goals to develop discharge plans during daily physician team rounds
- Develops a discharge plan in with collaboration of the family and the multidisciplinary health care team.
- Collaborates with health care professionals internally and regionally to promote a smooth transition to home.
- Coordinates appropriate medication, equipment, and/or home care services/supplies.
- Assists with follow-up appointments, activities, and schedules therapies.
- Meets with healthcare team to facilitate, coordinate, and evaluate patient’s discharge planning needs.
- Coordinates transfers to other healthcare facilities to ensure compliance with regulations, i.e. EMTALA.
- Offers education about level of care criteria and other payer information to members of the healthcare team.
Qualifications
Education
- Associates Degree in Nursing
License(s)/Certification(s)
- Current licensure in good standing to practice as a Registered Nurse in Iowa
- Successfully complete American Heart Association BCLS course within 90 days of hire date
Experience
- Minimum of two years clinical pediatric experience working with multidisciplinary teams.
Other
- Level 3 Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
