This is an excellent opportunity for an experienced home health Registered Nurse with strong OASIS expertise who enjoys combining clinical knowledge, quality review, clinician education, and collaboration.
As our OASIS Specialist (RN), you will partner closely with our clinical teams to review OASIS assessments, support plan-of-care development, strengthen documentation accuracy, and help ensure patients receive appropriate, high-quality care. You’ll serve as a trusted clinical resource to our field teams while helping Elevate Home Health maintain strong standards for quality, compliance, and patient outcomes.
This is a full-time remote position supporting our home health clinical operations in California.
What You’ll Do
- Review OASIS assessments for accuracy, completeness, clinical consistency, and alignment with current CMS OASIS guidance.
- Review patient information, including diagnoses, medications, procedures, clinical history, functional status, and ordered disciplines.
- Partner with clinicians to identify documentation opportunities and ensure OASIS assessments accurately reflect each patient's condition and care needs.
- Provide education and ongoing guidance to clinical staff regarding OASIS documentation and CMS requirements.
- Participate in start-of-care case conferences to review patient needs, appropriate disciplines, specialty pathways, and plan-of-care development.
- Assist clinicians in establishing appropriate short- and long-term goals, identifying priorities, and developing individualized plans of care.
- Review patient cases and clinical documentation to evaluate the appropriateness, quality, and effectiveness of services being provided.
- Provide clinical guidance, coaching, and feedback to strengthen documentation and care delivery.
- Participate in case conferences and collaborate with interdisciplinary team members to promote coordination and continuity of care.
- Conduct routine clinical record reviews and communicate findings, trends, and recommendations to agency leadership.
- Identify documentation and clinical trends that may impact quality outcomes, compliance, star ratings, or value-based purchasing performance.
- Support initiatives designed to improve clinical quality, documentation accuracy, patient outcomes, and organizational performance.
- Assist with clinical education and continuing education programs.
- Maintain current knowledge of OASIS guidance, CMS requirements, home health regulations, and accepted professional standards of practice.
What We’re Looking For
- Current, unrestricted Registered Nurse (RN) license in California.
- Graduate of an accredited school of nursing; BSN preferred.
- Minimum of two years of experience performing quality OASIS reviews in a home health setting.
- Current OASIS certification required prior to beginning the position.
- Strong working knowledge of current OASIS guidance and CMS home health requirements.
- Demonstrated knowledge or experience with CMS Star Ratings and Home Health Value-Based Purchasing (HHVBP).
- Experience reviewing clinical documentation and plans of care within a Medicare-certified home health environment.
- Homecare Homebase (HCHB) experience preferred.
- Strong clinical judgment and attention to detail.
- Ability to identify inconsistencies between patient presentation, clinical documentation, OASIS responses, and the plan of care.
- Ability to effectively educate, coach, and provide constructive feedback to clinicians.
- Excellent verbal and written communication skills.
- Strong organization, critical-thinking, and problem-solving skills.
- Ability to work independently in a remote environment while maintaining strong communication and collaboration with clinical and operational teams.
