Hospice Registered Nurse (RN) – PRN/Per Visit

We are seeking a compassionate, dependable, and motivated Hospice Registered Nurse (RN) to join our Hospice team on a PRN/Per Visit basis. This flexible position serves patients throughout San Antonio and surrounding communities, allowing you to enjoy a more adaptable schedule while making a meaningful difference in the lives of patients and their families. As a Hospice RN, you will provide skilled nursing care and end-of-life support to patients in their homes, assisted living communities, nursing facilities, and other care settings. You will work collaboratively with the interdisciplinary hospice team to ensure patients receive comfort-focused, compassionate care.

Responsibilities

Complete hospice admissions, assessments, and routine nursing visits Assess patient needs and identify changes in condition Develop, implement, and update individualized plans of care Manage pain and symptoms to maximize patient comfort and quality of life Administer medications, treatments, and therapies as ordered Educate patients, families, and caregivers regarding disease progression, symptom management, and end-of-life care Coordinate care with physicians, social workers, chaplains, hospice aides, facility staff, and other interdisciplinary team members Communicate significant patient changes promptly to appropriate team members Complete documentation accurately and within required timeframes Ensure compliance with hospice regulations, agency policies, and quality standards

Requirements

  • Current and valid Texas Registered Nurse (RN) license
  • Minimum of one (1) year nursing experience preferred
  • Hospice experience strongly preferred
  • Home health, palliative care, oncology, geriatric, or acute care experience is a plus
  • Strong assessment, communication, and critical-thinking skills
  • Ability to work independently in the field
