What You’ll Do

*

• Oversee and support a team of nurses, aides, and therapists
• Manage patient cases and develop individualized care plans
• Ensure high-quality care and compliance with NYS regulations
• Collaborate with physicians, families, and interdisciplinary teams
• Play an active role in quality improvement and staff development

*What We’re Looking For:*

• Active *NY RN license*
• *2+ years of nursing experience* (home health preferred)
• Leadership or case management experience is a plus
• Strong clinical, communication, and organizational skills

Pay: $81,067.52 - $97,629.70 per year Work Location: In person
