About this role
*Amada Senior Care of Palo Alto is looking for a compassionate, experienced Registered Nurse to join our team!*
This is a unique nursing opportunity focused on client assessments, care planning, caregiver education, quality assurance, and care coordination. Our RN plays an important role in helping seniors remain safe and supported at home while also working closely with families and healthcare professionals.
What You’ll Do

- Primarily conducting *long-term care assessments*.
- Support *Long-Term Care Insurance (LTCI) assessments and claims advocacy*
- Develop and update individualized *Plans of Care*
- Perform quality visits and monitor changes in client condition
- Provide training, guidance, and mentorship to caregivers
- Help caregivers understand and safely meet each client’s needs
- Communicate with families, physicians, case managers, social workers, and other healthcare professionals
- Identify changes in condition and recommend appropriate adjustments to care
- Document assessments and care plans in *AxisCare*
- Help maintain high standards of care and quality assurance

What We’re Looking For
*Required:*
- Active *California RN license*
- Strong clinical assessment skills
- Excellent communication and documentation skills
- Compassionate and professional approach
- Ability to work independently
- Comfortable working with seniors and their families
- Willingness to learn AMADA’s LTCI assessment and documentation processes
*Preferred:*
- Experience with senior care, home care, home health, hospice, or geriatrics
- Experience with care planning or patient assessments
- Experience educating or training caregivers
- LTCI experience
