*Job Overview* We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care in outpatient facilities. This role requires a strong ability to assess, diagnose, and treat patients, while also promoting health education and wellness initiatives. The Nurse Practitioner will work collaboratively with physicians and other healthcare professionals to ensure comprehensive patient care.
*Duties*
- Conduct thorough patient assessments, including medical histories and physical examinations.
- Diagnose and manage acute and chronic health conditions.
- Develop and implement individualized treatment plans based on patient needs.
- Provide health coaching to patients to promote healthy lifestyle choices.
- Collaborate with interdisciplinary teams in an outpatient rehabilitative setting.
- Maintain accurate medical records.
- Educate patients and families about disease prevention, treatment options, and post-care instructions.
