Join our Hospice Homecare team and make a meaningful difference in the lives of patients and families when they need compassionate care the most. We are seeking a dedicated Registered Nurse to provide after-hours hospice care and clinical support during weekend coverage.

  • Friday: 11:00 AM – 11:00 PM
  • Saturday: 7:00 AM – 7:00 PM
  • Sunday: 7:00 AM – 7:00 PM
  • Backup call required

Position Summary

The Weekend Hospice RN provides skilled nursing care and clinical support to hospice patients and families during weekends when the office is closed. This role is responsible for triaging after-hours calls, completing necessary visits, performing hospice admissions and starts of care when clinically appropriate, and ensuring patients receive timely, compassionate, high-quality care.

Key Responsibilities

  • Triage after-hours calls and assess patient needs.
  • Provide clinically appropriate home visits, including:
  • Symptom management visits
  • Crisis visits
  • Death visits
  • Other urgent, unplanned nursing visits
  • Complete hospice referrals, admissions, and starts of care during after-hours coverage when immediate hospice services are needed.
  • Deliver skilled nursing care according to the patient’s plan of care and physician orders.
  • Monitor patient conditions and communicate significant changes to the interdisciplinary team.
  • Coordinate care across settings, including inpatient, respite, and routine home care services.
  • Maintain accurate and timely clinical documentation.

Demonstrate professionalism, empathy, teamwork, and exceptional customer service in all patient and family interactions.

The Weekend RN will cover areas in Clarendon, Florence, Cheraw and Dillon.

Qualifications Required

  • Current South Carolina RN License
  • Current South Carolina Driver’s License and automobile insurance as required by law
  • Current BLS Certification
  • Active patient care experience within the past year
  • Minimum of 1-2 years of RN experience

Preferred

  • Hospice experience
  • Home Health experience
  • Palliative Care experience
  • Medical-Surgical nursing experience
