*Overview* Cornerstone Medical is a *direct primary care (DPC) clinic* committed to delivering thoughtful, patient-centered medicine to our local community. We are currently seeking a *Nurse Practitioner* to join our growing practice. *New graduates are encouraged to apply. *Our goal is to support our providers with longer appointment times, caring/supportive staff, and to elevate our care in such a way that excellence is what we are known for. *Responsibilities* * Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories to inform accurate diagnoses and treatment plans. * Manage acute and chronic conditions across specialties. * Administer medications and injections, including IV infusions following established protocols and safety standards. * Perform procedures such as suturing, catheterization, joint injections, I&D, wound care, and hormone pellet placement. * Utilize electronic health record (EHR) systems- Charm Health to document patient encounters accurately and efficiently. *We use Freed AI to reduce documentation burden and support clinical efficiency.* *Freed is an AI-powered medical scribe that helps generate clinical notes from patient encounters, allowing providers to spend less time charting and more time focusing on patients.* * Collaborate with physicians, specialists, and ancillary staff in both inpatient and outpatient settings to coordinate comprehensive care plans. * Provide telehealth consultations utilizing telemedicine platforms to extend care access and improve patient engagement. * Since our clinic does not bill insurance, we can focus on the patient, not the reimbursements. This is a model that truly puts the patient first. *Skills* * Proven experience working in diverse clinical environments including hospital medicine, emergency rooms, ICU/PICU units, nursing homes, assisted living facilities, or home care settings. * Strong proficiency with EMR/EHR systems familiarity with medical documentation standards is essential. * Excellent patient assessment abilities with a solid understanding of physiology and anatomy knowledge across age groups from neonatal to geriatric populations. * Understanding of functional medicine, proactive versus reactive medicine. Our clinic focuses on medication assisted weight loss, hormone replacement therapy for both men and women, primary care, chronic care, and care accross the lifespan. * Ability to perform triage effectively in urgent care while demonstrating critical thinking under pressure. * Competence in performing procedures like venipuncture (phlebotomy), IV insertion/infusion therapy. Knowledge of hormone pellet placement and joint injections a plus. * Strong communication skills for health coaching, patient education on medication administration or nutrition plans; experience working with individuals with disabilities or developmental challenges is advantageous. * Join a team that supports you while you support your patients.
