The Licensed Practical Nurse (LPN) in this part-time day shift position will provide essential nursing care within a hospital-based Skilled Nursing Facility (SNF). The primary goal is to deliver high-quality, compassionate care to patients recovering from acute illness or surgery, ensuring their safety and comfort throughout their stay. This role involves collaborating closely with registered nurses, physicians, and other healthcare professionals to implement individualized care plans and monitor patient progress.

The LPN will also play a critical role in patient education, assisting with medication administration, and documenting clinical information accurately. Ultimately, this position supports the facility’s mission to promote patient recovery and improve health outcomes in a supportive and professional environment.

Qualifications

Current and valid Licensed Practical Nurse (LPN) license in the state of employment. Completion of an accredited practical nursing program. Basic Life Support (BLS) certification. Demonstrated knowledge of nursing principles and practices relevant to skilled nursing care. Ability to communicate effectively with patients, families, and healthcare team members.

Experience working in a hospital-based Skilled Nursing Facility or similar healthcare setting. Certification in geriatric nursing or wound care. Familiarity with electronic health record (EHR) systems. Additional certifications such as IV therapy or medication administration endorsements. Strong organizational skills and ability to manage multiple patient care tasks efficiently.

Responsibilities

Administer medications and treatments as prescribed by physicians and according to facility protocols. Monitor and record patients’ vital signs, symptoms, and overall condition, reporting any changes to the healthcare team promptly. Assist patients with activities of daily living, including hygiene, mobility, and nutrition, while promoting independence whenever possible.

Collaborate with interdisciplinary team members to develop, implement, and update individualized care plans. Maintain accurate and timely documentation of nursing care, patient responses, and any incidents in compliance with regulatory standards. Provide emotional support and education to patients and their families regarding care plans and health management.

Ensure compliance with infection control policies and safety procedures to maintain a safe care environment.

Skills

The required skills enable the LPN to provide safe and effective nursing care by accurately administering medications, monitoring patient conditions, and documenting clinical information. Strong communication skills are essential for coordinating with interdisciplinary teams and educating patients and families about care plans.
