Registered Nurse Case Manager

• In-home community-based nursing case management.
• Conduct thorough patient assessments, including vital signs, medical history review, monitoring and following up, working with collateral contacts to determine care needs.
• Develop and implement individualized care plans in collaboration with healthcare professionals, ensuring alignment with health regulation policies and best practices.
• Manage patients across various settings such as hospital medicine units, emergency rooms, intensive care units (ICUs), pediatric units, geriatrics, behavioral health facilities, and home health environments.
• Coordinate discharge planning and facilitate smooth patient transitions while optimizing resource utilization.
• Utilize Electronic Health Records (EHR) systems for accurate documentation of medical records, patient interactions, case management activities, and compliance with HIPAA regulations.
• Collaborate effectively with physicians, nurses, social workers, and other healthcare providers to ensure comprehensive patient service and optimal clinical outcomes.

*Skills*

• Extensive critical care experience with a strong background in ICU or emergency medicine settings.
• Proven experience working within Level I or Level II trauma centers; familiarity with acute trauma protocols is highly desirable.
• In-depth knowledge of hospital medicine, medical-surgical nursing, and outpatient clinic operations.
• Strong understanding of health regulation policies including HIPAA compliance and medical terminology proficiency.
• Skilled in patient assessment techniques, vital signs monitoring, diagnostic evaluation, and clinical case management strategies.
• Proficient in EMR/EHR systems for managing electronic health records efficiently while maintaining data security standards.
• Excellent client communication skills with the ability to educate patients about their treatment plans and discharge instructions effectively.
• Experience managing diverse populations including pediatrics (PICU), geriatrics, behavioral health patients, and those requiring hospice or home health services.
