Summary
The Clinical Coordinator is responsible for supporting and leading dedicated nursing staff . The Clinical Coordinator ensures staff assignments provide care that enhances residents' ; collaborates with medical providers; assists with staff development; and oversees clinical operations leading to positive rehabilitation outcomes.
The schedule for this position will be Monday-Friday on days with every other weekend required.
Pay is commensurate with years of clinical and leadership experience. Generous extra pay for hours worked past 3:00 pm and on weekends.
Qualifications /
Education
- Current RN a minimum of one (1) year of long term care or acute care experience required
- Minimum of one (1) year formal leadership experience in health care required
- Previous Rehabilitation experience highly desirable
- Current CPR certified required
- Excellent communication skills
- Willingness to support and lead others to living St. John’s Brand Characteristics
- Compliance with NYS Department of Health immunization requirements.

- Sits, stands, walks most of the day. Frequent moving and lifting of residents. Moderate to heavy lifting (up to 50 lbs).

Responsibilities
- Responsible for coordination of clinical services for each resident
- Oversees the development of individualized care plans
- Generates nursing staff assignments, assuring follow through including complete and accurate written and verbal communication to support the care needs of each resident
- Establishes a compassionate environment by providing emotional, psychological, social and spiritual support to residents, friends, and families
- Assists floor nurses with bedside care when needed, performs periodic rounds on floors and assists with Nursing Department audits
- Ensures that all staff embraces and lives the St. John’s Brand Characteristics: Friendly, Respectful, Responsive, Compassionate, Innovative, and Fun as it applies to their duties as a St. John’s employee
