Position Type: Part-Time / PRN / Part Time (Flexible)Location: Travel to clients’ homes within the service area (Guilford and Davidson Counties)

Job Summary

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to travel to clients’ homes to complete start-of-care admissions, required 90-day reassessments and nursing duties. The list of nursing duties that the agency provides will be provided at the interview or upon request. This role is ideal for an RN who enjoys autonomy, flexible scheduling, and working directly with clients in a home care setting.

Responsibilities

Conduct in-home client admissions/start-of-care assessments

Perform 90-day Reassessments And Additional Assessments As Required

Complete accurate and timely clinical documentation

Assess client health status, functional abilities, and care needs

Collaborate with office staff, caregivers, and families to support care plans

Identify changes in condition and communicate concerns appropriately

Ensure compliance with company policies, state regulations, and best practices

Qualifications

Active Registered Nurse (RN) license in good standing

Minimum of 1 year nursing experience (home care or community-based experience preferred)

Strong assessment, documentation, and communication skills

Ability to work independently and manage time effectively

Reliable transportation and willingness to travel to client homes

Comfortable using electronic documentation systems

Schedule & Compensation

Flexible scheduling

Competitive per-visit or hourly pay (based on experience)

Mileage reimbursement (if applicable)
