Summary

We are seeking a dedicated and experienced *Primary Registered Nurse (RN) – Case Manager* for a 13-week contract assignment in Montrose, CO. The RN Case Manager will be responsible for coordinating patient care, ensuring continuity of services, and working collaboratively with interdisciplinary teams to achieve optimal patient outcomes. This role primarily operates on a weekday schedule with occasional on-call and weekend coverage as needed.

• Coordinate and manage patient care plans across the continuum of care
• Perform comprehensive patient assessments and develop individualized care plans
• Collaborate with physicians, healthcare providers, and support staff to ensure quality care delivery
• Monitor patient progress and adjust care plans as necessary
• Facilitate discharge planning and ensure appropriate follow-up care
• Educate patients and families on treatment plans, medications, and healthcare needs
• Maintain accurate and timely documentation in accordance with facility policies
• Participate in on-call rotation and provide weekend back-up coverage when required

Requirements

*

• Active *Registered Nurse (RN) license – Colorado (or compact state)*
• Minimum *1–2 years of RN Case Management or relevant clinical experience preferred*
• Strong knowledge of care coordination and discharge planning
• Excellent communication and organizational skills
• Ability to work independently and as part of a multidisciplinary team
• BLS certification (preferred/required as per facility guidelines)
