Hospice - Full-Time Licensed Vocational Nurse (LVN)

Location: Amarillo, TX, 79106

High Plains Senior Care Hospice

We are seeking a skilled and compassionate Full-Time Hospice LVN to join our team. The Hospice LVN will be responsible for providing hospice care to patients in their homes, under the direction of a registered nurse or physician. The ideal candidate will have experience in hospice care, excellent communication skills, and a passion for helping others.

Responsibilities

  • Assess patient's health status and develop care plans
  • Administer medications and treatments as prescribed by a physician
  • Monitor patients' vital signs and report any changes to the RN or physician
  • Provide wound care and other nursing interventions as needed
  • Document patient care and progress in electronic medical records
  • Communicate with patients, families, and other healthcare providers as needed
  • Participate in interdisciplinary team meetings and case conferences
  • Adhere to all state and federal regulations and agency policies and procedures
  • Qualification & Requirements
  • Current LVN license in the state of Texas
  • Minimum of one year of experience in hospice care (preferred)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong critical thinking and problem-solving skills
  • Valid driver's license and reliable transportation
  • Ability to pass a background check and drug screening
