About the Role

New Approach Healthcare Inc is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Stoughton, MA. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality home care services to Adult Foster Care and Group Adult Foster Care (AFC/GAFC) patients across diverse settings. The ideal candidate will possess a strong clinical background, excellent patient assessment skills This is an exciting opportunity to make a real difference in patients' lives while working in a supportive, patient-centered environment. Responsibilities

Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation skill. Monitor patients' conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns appropriately through agency procedures. Assist with activities of daily living such as mobility support, and basic life support during emergencies.

Collaborate with physicians, case managers, and caregivers to develop and implement individualized care plans aligned with long-term care regulations. Maintain meticulous medical documentation in compliance with HIPAA and clinical confidentiality policies. Educate patients and their families on health management strategies, including dementia care, Alzheimer's support, and medication administration.

Requirements

  • Active LPN license in the state of Massachusetts (required)
  • Graduate of an accredited Licensed Practical Nursing program
  • Current BLS/CPR certification
  • Previous clinical experience in a healthcare setting preferred
  • Proficiency with electronic medical records (EMR) systems
  • Strong communication, organizational, and critical thinking skills
  • Compassionate, patient-focused demeanor with a commitment to quality care
