The Licensed Practical Nurse (LPN) is responsible for coordinating and supporting the individualized service plans for community residents. The role supervises, and provides leadership and direction to, community team members to ensure quality of care and services are provided. Requirements for the Licensed Practical Nurse (LPN): Current, valid Practical Nurse License in good standing from the state of Pennsylvania. Experience as a licensed nurse preferred. Experience in Personal Care or Assisted Living preferred. Knowledge of DHS regulations preferred.
