How you'll make an impact in this role

  • Deliver compassionate, high-quality direct patient care that strictly adheres to established, patient-first best practices and organizational protocols.
  • Manage the patient's comprehensive care journey, meticulously documenting progress and communicating critical status updates to the multidisciplinary team to ensure continuity and safety of care.
  • Coordinate patient services across all clinical disciplines, acting as the primary liaison to guarantee seamless and well-integrated care for optimal outcomes.
  • Execute physician and nursing orders accurately and promptly to ensure the timely advancement of patient treatment plans.
  • Facilitate comprehensive discharge planning by proactively assessing patient needs and collaborating with the healthcare team to ensure a safe, smooth, and successful transition home.

Licensure / Certification / Registration

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
