About this role

The RN-Hospice Home Care delivers specialized care to patients and their families, helping them navigate end-of-life journeys with respect and peace in familiar surroundings. The RN-Hospice Home Care partners with the interdisciplinary team, including physicians, social workers, and spiritual care providers to create and implement individualized care plans tailored to meet each patient's unique needs. Works directly in the patient's home to perform direct and indirect total nursing care for the Hospice patient and family in accordance with the St. Francis Hospice Standards for Nursing Practice in the home setting; instructing the family in simple nursing procedures in accordance with the Hospice Interdisciplinary Plan of Care.
