About this role
Sangre de Cristo Community Care is seeking a Float RN to provide clinical support across our program. In this dynamic role, you'll help ensure continuity of care by assisting with admissions, patient visits, and providing coverage for RN Case Managers during PTO, education, and other staffing needs. Responsibilities include completing hospice admissions and start-of-care visits, providing routine and PRN patient visits, covering RN Case Manager caseloads, assisting with symptom management, collaborating with physicians and interdisciplinary team, educating patients and families, documenting patient care in EMR, and maintaining continuity of care across the team.
