Registered Nurse (RN) – Transitional Care Management Clinic Location: Boardman, OH (Main Campus) Schedule: Full-time. Monday – Friday. Day shift. The Role You will assist patients recently discharged from acute care. You will gather clinical data and help prevent hospital readmissions. Key Responsibilities: Patient Assessments: Conduct post-acute phone assessments. Identify barriers to patient recovery. Care Coordination: Review discharge instructions, medications, and referrals. Scheduling: Facilitate and schedule face-to-face follow-up visits. Documentation: Log all patient calls accurately in the EMR system. Compliance: Maintain patient privacy and strictly follow regulatory standards (OSHA, ODH). Qualifications License: Active Ohio RN license. Education: BSN degree is preferred.
