What You'll Do
- Provide scheduled and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing facilities.
- Assess patient needs and provide symptom management, medication education, wound care, and comfort-focused nursing interventions.
- Educate and support patients and families throughout their hospice journey.
- Collaborate with the interdisciplinary team to ensure individualized, compassionate care.
- Complete timely and accurate EMR documentation.
What You'll Bring
Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred, with consideration given to nurses from home health, oncology, ICU, palliative care, or other end-of-life care settings .
- Outstanding communication and relationship-building skills.
- Ability to work independently while managing a field-based schedule.
- Reliable transportation and a valid driver's license.
