Hospice Registered Nurse - Part Time

We are seeking a compassionate and dedicated Registered Nurse to join our Hospice Team in the Lewistown, PA, and Surrounding areas. The ideal candidate will have a strong clinical background and a passion for providing exceptional end-of-life care. Our team is committed to providing personalized support and comfort in the familiar surroundings of patients' homes.

Responsibilities

What You'll Do

Conducting Start of Care comprehensive assessments Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete Preparing accurate, orderly, and timely documentation Coordinating care with patient and provide proper reports to Case Manager Provides health care instructions to the patient/family as appropriate per assessment and plan

Qualifications

About You

An active RN license in the state in which practicing (Minimum of 1 year of experience preferred) Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills Compassionate and empathetic approach to patient care Ability to work independently and as part of a team Knowledge of hospice regulations and standards of care Reliable transportation and willingness to travel
