Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must possess a valid state driver license. Must maintain automobile liability insurance as required by law. Must have basic demonstrated technology skills, including operation of a mobile device. Medicare home health or hospice experience is preferred. Must have dependable transportation in good working condition and be able to safely drive an automobile in all types of weather conditions.
