Human Development, (CHD) is seeking a Registered Nurse - RN for our

Adult Community Clinical Services (ACCS), located in Holyoke, MA. ACCS is funded through the Department of Mental Health and is a multidisciplinary outreach approach for persons served in need of psychiatric and other services living independently in the community.

This involves active engagement including assertive outreach, consistent assessment and treatment planning, skill building, symptom management, addiction treatment, and behavioral and physical health monitoring and support. YOUR ROLE AS A REGISTERED NURSE: ACCS teams include licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The Team works collaboratively to provide person-centered, evidence-based services in support of

Persons\' treatment goals. The RN coordinates those functions and activities that provide for, maintain and promote the health of Persons served. The RN provides direct support to Persons in order to assess needs, participates in medical reviews, and provides counseling in areas of health and psycho-physiological well-being, and liaisons with other medical and community-based providers. This is an administrate RN role.

REQUIRMENTS: ? Bachelors preferred ? Mass Registered Nurse License required ? Two years of experience working within mental health and/or substance use field is preferred. ? MAP experience preferred ?

Residential experience preferred ? Acceptable Driving record SUCCESS
