*Community Healthcare Partners* is seeking a *dedicated and experienced APRN/NP* to provide high-quality, hands-on care in *long-term care facilities throughout Lincoln and surrounding areas, including Fremont*. Mileage reimbursement included.
This is an ideal opportunity for a confident provider with *primary care experience* who thrives in a collaborative, fast-paced environment and values building meaningful patient relationships.
*Position Highlights*

- *$65–$75/hour (Part-Time)*
- *Full-Time Potential:* $90,000 base + *uncapped bonus structure* (earning potential $150,000+)
- *Facility-Based Rounding (Onsite – Not Remote)*
- *Mileage Reimbursement Provided*
- *Start Date Upon Credentialing*
*\*\*Schedule Requirement\*\*:* *Every Weekend (Saturday & Sunday)* – continuity of care is essential for this role.
*Key Responsibilities*
- Perform physical exams and review medical histories
- Order, interpret, and manage diagnostic testing
- Diagnose and treat acute, chronic, and infectious conditions. Emphasis on Geriatric Chronic care conditions.
- Prescribe and manage medications
- Educate patients and families on care plans and prevention
- Maintain accurate, timely EMR documentation (EClinical Works)
- Collaborate with interdisciplinary team
- Promote proactive, patient-centered care and outcomes
*Qualifications*
- Licensed *APRN / Nurse Practitioner in Nebraska*
- *Primary Care experience required (credentialed provider preferred)*
- Strong clinical judgment and diagnostic skills
- Knowledge of healthcare regulations and compliance standards
- Excellent communication and patient education abilities
- Team-oriented, dependable, and compassionate
